Change Request Form
Parents/Guardians looking to notify RSCO Transportation of an address change must contact your child's school or program.  DO NOT SUBMIT THIS FORM.INSTRUCTIONS:
  • Please complete the fields below and submit the form.  Required fields are highlighted.
  • After the form is submitted, a confirmation page will be displayed with a Ticket ID that you should use when communicating with CREC Transportation.
  • Only include information for one student per form. You can submit the same form for another student by clicking 'Add Another' on the confirmation page.

    Reason CodeDescription
    "School or Program Use Only": Add StudentSchool or Program only, is submitting request to add student.
    Bus Stop ChangeRequest submitted to changes to established pickup or drop-off locations.
    RSCO Transportation does not provide door-to-door transportation.
    Generally, centralized bus stops are not within walking distance of a student’s home.
    Bus Stop Change- Alternate StopBus Stop Change for Alternate Stop; AM vs. PM or because of court ordered joint custody requirements (must submit court order) or Day Care Stop
    Contact Info ChangeChange(s) submitted for contact information; contact name(s), phone numbers, email, and other contact information
    Home Stop Request - "OTHER"Requesting a home stop for reason other than medical condition. (Does not guarantee a home stop approval)
    Home Stop Request- "MEDICAL"A home stop request requires medical documentation with students specific health information to be on file or submitted to CREC.
    Joint Custody RequestFamilies who have joint custody require more than one bus stop.
    Possible Safety IssueRequest to change existing stop because of a perceived safety issue. Requires specific information of why stop is considered unsafe.
    Remove Student from SchoolStudent no longer attends this school.
    Remove Student from TransportationStudent is Transported to School by other.
